Cena previously underwent a hair transplant in November 2024 after fans drew attention to his thinning hair. He reflected on his initial hesitations, telling People last year, “As I was trying to hide my hair loss, the audience was bringing it to light. I saw their signs that said ‘The bald John Cena’. They pushed me into going to see what my options were.” He outlined his current hair care routine, which includes red-light therapy, minoxidil, vitamins, shampoo, and conditioner, in addition to the transplant.
“I hate the fact that if there wasn’t so much shame around it, I’d have gotten it done 10 years ago. I thought I was alone, but seven or eight out of 10 [men] suffer from thinning or baldness.”
Describing the transplant process, Cena explained that it involves moving hair follicle by follicle from one area to another. He found the initial procedure to be life-changing, stating, “It completely changed the course of my life.”
This upcoming second procedure involves FUE treatment, and Cena expressed his gratitude to Dr. Ken Anderson and the staff at the Anderson Center for Hair for their support. He believes that having a fuller head of hair could lead to more acting roles.

Cena has also spoken about the challenging side effects of the surgery, including significant swelling that he described to TMZ as giving him an 'alien head.' Despite the discomfort, which included swollen wounds and a temporarily altered head shape during the initial 14 days post-surgery, he maintained that he did not care and returned to work promptly.
